Yes you can, but it's a steep learning curve.
You have two options; 1. Design using standard osCommerce - this way you get to learn osCommerce inside out which is always a good thing. 2. Use "STS" (Simple Templating System) - allows you to design your whole site inside 1 HTML file - using "placeholders". This is easier that #1, but you don't learn even half as much about osCommerce. See the Top 100 osCommerce sites list at http://www.clubosc.com/hot100/ - these are a mix of STS and non-STS osCommerce sites - should give you some inspiration.
